Student Emergency Information Form

This form provides the school with the required contact information for your child. It must be completed at the beginning of each school year (or whenever there is a change in information) and provided to the school.

Acceptable Use Policy (AUP) For District Computer and Network Systems

All uses of the Los Angeles Unified School District (LAUSD) computer and network systems by students, employees, contractors, and others are subject to the LAUSD’s Acceptable Use Policy (AUP).
 
 
 

Student Enrollment Form

Parents/guardians/caretakers wishing to enroll a child for the first time in a Los Angeles Unified school should download the Student Enrollment Form, complete electronically, print, sign and take the form to the school.
 
 

Parents' Bill of Rights & Responsibilities

Parents as Equal Partners in the Education of Their Children, a resolution adopted by the LAUSD Board of Education in December 2010, embraces family strengths and assets as essential to the academic success of students, and recognizes parents as the first and most important lifelong teachers of their children. To that end, families and schools accept responsibility for student success and commit to a partnership that:

  • Maintains high expectations for student achievement
  • Ensures all children are college and career ready
  • Promotes productive conversation and collaboration
  • Reflects mutual respect and support for each other
